Everyone 18 years and older has the right to be fully informed about their health care and to make choices based on their goals, values, and beliefs. On too many occasions, families’ lives are torn apart when unexpected or expected end-of-life happens at any age or stage without the proper planning. Learn how to put documents in place that will inform the medical community and your loved ones (and remove the burden from them) by creating a health care plan. We’ll talk about what a health care plan is, what forms and documents are involved, which forms are legal and which are for informational purposes, and with whom to communicate your plan.

Caren Silverlieb

Caren Silverlieb received her master’s degree in the Management of Human Services with a concentration in Aging from the Heller Graduate School at Brandeis University. Caren has held executive and management positions in government-funded, non-profit and private community-based long-term care programs, affordable housing, assisted living, and her own business - SilverLiving Consulting. Caren is a frequent presenter at state and national conferences, private companies, universities, and the public. Currently, she serves on the Board of Trustees and the Community Advisory committee of the Newton Community Development Foundation.
